The Patrick County Health Department offering Back to School Immunization Clinic

The Patrick County Health Department is offering some extra Back To School immunization clinics over the next few months for rising kindergartners and rising 7th & 12th graders. The clinics are Fridays from 1:00 p.m. - 3:30 p.m. by appointment on these dates: March 10, April 14, and May 12. If you are unable to attend one of the Friday Back-to-School clinics, appointments can also be made on Mondays from 8:30 a.m. - 11:15 a.m. and 1:00 p.m. - 3:00 p.m. A parent or legal guardian must be present at the time of service if the child to be vaccinated is under the age of 18. If insured, bring insurance card at the time of service. If the child to be vaccinated has received vaccines in another state other than VIRGINIA, please bring a copy of their immunization record to the appointment.
